Proof of Address :
For Proof of Address customer has to submit a self-attested copy of any one of the following documents: Telephone Bill/ Electricity Bill/ Municipality Tax Bill/ Water Bill / Ration Card/ Passport/ Voter ID Card/ Driving License.
Proof of Photo Identity* : For Proof of Photo Identity the customer has to submit a self attested copy of any one of the following documents: Passport / Voter ID/ Driving License/ Income Tax PAN / Arms License/ Photo Identity care issued by Government or Statutory Authority.
PAN/GIR number. In the absence of PAN/GIR number, declaration is in Form60 / Form61 is to be furnished.
Mandate forms have to be submitted for Exemption of Security Deposit. Three types of Mandate forms are there :
a. BSNL Land Line Subscribers of Calcutta Telephone District : The telephone should be owned by the subscriber himself and should be a number of Calcutta Telephones only. A photocopy of last paid bill and Mandate forms have to be submitted duly signed by the subscriber. The subscriber will get STD/ Roaming facility if the land line is having STD facility. If subscriber already enjoying free security for 'TARANG' (WLL-M) against the same number should not apply.
b. Government/Public Sector Units/Bank Employees : Mandate forms have to be submitted duly signed by the subscribers as well as certified by the controlling officers.
c. BSNL Employees : Mandate forms have to be submitted duly signed by the subscribers as well as certified by controlling officers.
