" AO, BSNL, CALCUTTA TELEPHONES". Duly filled application form has to be submitted to The DE (INTERNET ), 2nd floor,Telephone Bhawan, 34, B.B.D.Bag (S), Kolkata-700 001 for technical advice & guidance. |
| 3. | Commercial formalities : Receipt of application, issue of demand note, works order etc. will be issued by CO (LD-I), 6th floor, Telephone Bhawan, Kolkata -700 001. |
| 4. | Initial payment will be made to AO (CASH)/CS, BSNL, 1st floor, Telephone Bhawan,
34 B.B.D.Bag (S), Kolkata-700 001. |
| 5. | Modems as per requisition are to be supplied by the customer only. |
| 6. | For allocation of IP addresses the customer has to collect the Form-III from
O/O DE (INTERNET) after submission of application form. (Click here for FORM III)

| 7. | Duly filled up form-III along with network layout diagram at customer end has to be submitted to DE (INTERNET). |

| 9. | Leased Line application for recognized educational institutions, Govt. organizations, Newspapers & Newspaper agencies should be accompanied with necessary supporting documents for their organization's credentials. |
