Commercial formalities : Receipt of application, issue of demand note, works order etc. will be issued by CO (LD-I), 6th floor, Telephone Bhawan, Kolkata -700 001. |

Modems as per requisition are to be supplied by the customer only. |

For allocation of IP addresses the customer has to collect the Form-III from
O/O DE (INTERNET) after submission of application form. (Click here for FORM III)

Duly filled up form-III along with network layout diagram at customer end has to be submitted to DE (INTERNET). |
