MANDATE   FORM FOR NEW MOBILE CONNECTION WITHOUT SECURITY DEPOSIT

: Security Deposit for BSNL CELLULAR  MOBILE SERVICES, CTD

 

 

 

1.

 

I am a bonafide customer of BSNL Landline /PSTN under the jurisdiction of Calcutta Telephones  District (CTD) 

having a telephone Number ……………….. in my own name, installed at 

………………………………………………………………………………………………………………………………………

………. 

(The self attested photo copy of last paid bill is enclosed). 

 

2.

 

I hereby give a mandate to BSNL allowing existing deposit against the above mentioned PSTN telephone for 

realization of outstanding, at the full discretion of BSNL, against the cellular mobile telephone subscribed by me. 

3.

 

(a) I hereby give a mandate to BSNL allowing for disconnection of the above PSTN telephone, at the full discretion 

of BSNL,  in case of any default on my behalf in payment of bills against the cellular mobile telephone subscribed by 

me.  

(b) In case of any default on my behalf in payment of bills against the above mentioned PSTN telephone, BSNL, 

Calcutta Telephones shall have the full right to disconnect my cellular mobile telephone also. 

 

4.

 

The category of the cellular mobile telephone (i.e. local or STD) will be same as that of the above mentioned PSTN 

telephone.  

 

5.

 

(a) It is obligatory on my part to intimate the BSNL Mobile Services, Calcutta in writing regarding any change in 

category, or number of the above mentioned PSTN telephone. 

 

(b) It is obligatory on my part to intimate the BSNL Mobile Services, Calcutta in case of change of address of the 

above mentioned PSTN telephone and sign a new mandate form indicating the change. 

 

6.

 

I understand that in case ISD is required in my mobile connection, then Security Deposit is payable as per existing 

tariff for cellular mobile connection of BSNL Mobile Services, Calcutta. 

 

7.

 

I understand that I may be required to be deposit additional Security Deposit at a rate decided by BSNL in times to 

come.  

 

8.

 

I have

 

not availed myself of this facility for any other GSM mobile or CDMA mobile ( Tarang) connection from 

BSNL. I will be totally responsible for the usage of this mobile connection and payment of bills against this 

connection. 

 

9.

 

In case of surrender of my above mentioned PSTN telephone or change of ownership, it is obligatory on my part to 

intimate the same to BSNL Mobile Services, Calcutta in writing and Security Deposit as applicable for my cellular 

mobile connection will be required to be paid by me.
