1. Application forms are available at all customer service centers of Calcutta Telephones & O/o DE(Internet) at 2nd floor Telephone Bhawan. The form is also available in the web site.
2. Duly filled application form has to be submitted to Divisional Engineer (Internet), 2nd floor,Telephone Bhawan, 34, B.B.D.Bag (S), Kolkata-700 001 
3. Commercial formalities : Receipt of application, issue of demand note, works order etc. will be issued by CO (LD), 3rd floor, Telephone Bhawan, Kolkata -700 001.

Payment will be made to AO (CASH)/CS, BSNL, 1st floor, Telephone Bhawan, 34 B.B.D.Bag (S), Kolkata-700001.
All payments are to be made through DD / Banker's cheque in favor of " AO, BSNL, CALCUTTA   TELEPHONES"
5. For Renewal of the Account , Same Application form ( duly filled ) has to be submitted to Divisional Engineer (Internet), 2nd floor Telephone Bhawan 34, BBD Bag (S) Kolkata-700001.
