| 2. | Duly filled application form has to be submitted to Divisional Engineer (Internet), 2nd floor,Telephone Bhawan, 34, B.B.D.Bag (S), Kolkata-700 001 |
| 3. | Commercial formalities : Receipt of application, issue of demand note, works order etc. will be issued by CO (LD), 3rd floor, Telephone Bhawan, Kolkata -700 001. |

Payment will be made to AO (CASH)/CS, BSNL, 1st floor, Telephone Bhawan,
34 B.B.D.Bag (S),
Kolkata-700001. All payments are to be made through DD / Banker's cheque in favor of " AO, BSNL, CALCUTTA TELEPHONES" |
| 5. | For Renewal of the Account , Same Application form ( duly filled ) has to be submitted to Divisional Engineer (Internet), 2nd floor Telephone Bhawan 34, BBD Bag (S) Kolkata-700001. |
